Vendor Information and EFT form

Please fill out the following vendor information form so we may process your invoice payments.  

If you'd like payments by electronic funds transfer (EFT) please complete the EFT questions below otherwise you will receive a check by postal mail to the address you provide.  

Submission of this forms indicates your acceptance of the following authorization: 

"I hereby authorize NLYSF and/or its payment processor to directly deposit invoice payments in the bank account listed below. This authorization is to remain in force until the depositor has received written authorization from me of its termination or change.  Also, I grant OneWealth the right to correct any Electronic Funds Transfer resulting from an erroneous overpayment by debiting my account to the extent of such overpayment."
